This document was created to help the golf professional/account save time by only having to fill out one credit application to open credit accounts with almost all our participating vendors.

All members will print and fill out credit application in its entirety.  Once credit application is filled out member will then keep it on file.  When member decides to open an account with a vendor they will fax the Universal Credit Application to vendor.  Vendor will use information that is on application to establish credit line amounts.  Some vendors will then fax member a one page guaranty document for member to sign.  Once all credit needs are completed member will be eligible to order merchandise from participating vendors.
